Inspector Notes
An investigation occurred on 11/02/2022 and 11/16/2022 in response to a self reported incident regarding behavioral guidance received on 11/1/2022. Based on staff and parent interview, children and staff record reviews, and observation, the preponderance of evidence supports a violation and the area of non-compliance is identified on the violation notice. If you have any questions you may contact me at jessica.willis@dss.virginia.gov or 571-352-1453.
