Inspector Notes

An unannounced monitoring inspection was conducted on 11/2/2022 and 11/16/2022. There were 52 children directly supervised by 13 staff. The physical plant, 6 staff records, 5 children?s records, medications, evacuation drills, injury reports, and emergency supplies were inspected. Children were observed eating lunch, listening to an audio book, and engaging in free play. Naptime was also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 6 staff records did not contain a criminal history record information check from any state in which the individual has resided in the preceding 5 years, prior to the first day of employment.

Evidence:
1. Documentation in the record for Staff #3 (date of employment 2/8/2022) indicated they resided in Washington D.C. in the preceding 5 years before employment.
2. The record for Staff #3 did not contain the results of a criminal history record check for Washington D.C.
Plan of Correction: The center will provide documentation that the criminal history results were requested within 10 business days.

Based on record review, 1 of 5children's records did not contain documentation of immunizations for a child prior to the child's first attendance.

Evidence:
1. Child #5 began attending the center on 9/19/2022.
2. The record for Child #5 did not contain documentation of immunizations.

Plan of Correction: A new form was created for the parents to complete and sign. The center will request the information from the parents.
