Inspector Notes

An unannounced renewal inspection was conducted on 4/25/2024 between the hours of 9:30am-12:00pm. 46 children were observed directly supervised by 10 staff. The physical plant, 4 staff records, 5 children?s records, medications, evacuation drills, emergency supplies, and policies were inspected. Children were observed eating lunch, looking at books, and playing quietly. Hand-washing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 5 children's records reviewed did not contain all required information.

Evidence:
1. The record for Child #4 did not contain the address for two designated people to call in an emergency if a parent cannot be reached.
2. The state of residence was provided for one emergency contact. The city of residence was provided for one emergency contact.
Plan of Correction: The parent was contacted during the inspection to obtain the additional information.
