During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 01/21/2025, as a part of the licensure period. The on-site inspection began at 11 am and ended at 3 pm. The inspector reviewed compliance in the areas listed above. There were 64 children present and 11 staff. The inspector reviewed 6 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The center is required to obtain a completed sworn statement prior to the employee's first day of employment. A sworn statement for Staff #1, who has been employed for two months, was not available for review.

Centers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #1, who had been employed for two months, did not have a completed national criminal background check available for review.

The center must request a search of the central registry prior to the employee's first day of employment. Staff #1, who had been employed for two months, did not have documentation of a central registry search requested.

The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not have documentation of a a sex offender registry check, and documentation of a search request of the child abuse and neglect registry from one state for Staff #1, who has been employed for two months.

Standard 8VAC20-780-130-A
The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. The immunization documentation for Child #1 was obtained 3 weeks after Child #1 began attending the center.
Plan of Correction: A current record of immunizations are in the file for Child #1. The center will ensure immunization records for children are obtained prior to enrollment.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. The TB screening for Staff #2 was dated eleven months prior to date of employment. Staff #2 has been employed for 4 months.
Plan of Correction: Staff will complete a new TB screening.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening. The updated screening for Staff #4 was due seven months ago and the updated screening for Staff #5 was due two months ago. The updated screenings have not yet been completed.

Standard 8VAC20-780-270-A
The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Peeling paint was observed on a wall below the interior window, accessible to children in the Twos room. Chipped paint debris was visible on the floor below the area of peeling paint on the wall.
Plan of Correction: The area of peeled paint will be covered today. Administration will speak to a supervisor to request a work order.

When centers choose to provide meals or snacks, centers must provide a menu listing foods to be served for meals and snacks during the current one-week period which shall include all required elements. The menu posted in the center was two weeks old.
Plan of Correction: The center will print and post the current menu.
Standard 8VAC20-780-70
Centers are required to maintain a record for each staff person which includes all required information. A staff record for Staff #1, who has been employed for two months, was not available for review. Staff reported information was stored electronically; however staff did not have access to the information.
Plan of Correction: Administration will contact supervisor to request the staff record.
