During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 3/12/2025 and completed on 4/2/2025 in response to a complaint received by the licensing office on 2/26/2025 relating to behavior guidance and forbidden actions. The inspector reviewed 1 child record and 1 staff record on-site. This inspection included document review, tour of the facility, interviews, and observations. The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to administration and staffing and supervision; and are documented on the violation notice issued to the program.
Standard 8VAC20-780-150-B
Immunization reports obtained for the child record must be signed by a physician, designee or official of a local health department. The immunizations in the record for Child #1 did not contain the signature of a physician, designee, or health department official.
Plan of Correction: The center will ensure immunization documents are signed by a doctor.
Standard 8VAC20-780-350-B-3
The required ratio for 2 year old children is one staff per eight children. On 2/25/2025, Staff #3, who was assigned to a classroom in the enclosed outdoor play area, asked Staff #1 to stand in the doorway of the 2year old classroom facing outside to provide supervision to the outdoor classroom while Staff #3 went to the restroom. During this time, there were 9 children in the 2 year old classroom, supervised by Staff #2, leaving the 2 year old classroom out of compliance with the required 1 staff per 8 children ratio. During this time, Child #1 ran from the 2 year old classroom, outside to the play area and had to be retrieved by Staff #1 and returned to their assigned classroom.
Plan of Correction: Teachers are to maintain ratio at all time. If a teacher needs to go to the restroom or leave the classroom for any reason, they must use the radio to request assistance. The policy was reviewed with all staff.
Standard 8VAC20-780-40-G
The center must require that the person enrolling the child present proof of the child's identity and age. If the parent, guardian, or other person enrolling the child does not provide the information required within seven business days of initial attendance, the center shall immediately notify the local law-enforcement agency in its jurisdiction of such failure to provide the requested information.
The center did not view proof of the child's identity and age for Child #1 within seven business days of attendance and did not notify local law enforcement of failure to provide the requested information. Child #1, who was present on the day of inspection, had been attending the center for more than one month.
Plan of Correction: The center requested and reviewed the information provided by the parent on the date of the inspection. The center will ensure a valid proof of identity and age is viewed no later than seven business days from the first date of attendance when a child is enrolled.

The record for Child #1 did not contain employment information for one parent.
