Inspector Notes

An unannounced inspection was initiated and completed on 05/08/2025 in response to a complaint received by the licensing office on 05/1/2025 relating to behavioral guidance. The inspector reviewed 4 children?s records on-site. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-400-C
When used as a behavior guidance technique, time out must meet all requirements. Time out shall not be used with infants or toddlers. On the evening of 4/29/2025, Child #1, age 17 months, was placed in a crib for an unknown duration of time. Staff reported Child #1 was placed in the crib due to biting another child.
Plan of Correction: Management reviewed discipline policy with staff.
Standard 8VAC20-780-550-P
Centers are required to maintain a written record of children's serious and minor injuries in which entries are made the day of the occurrence. Staff reported Child #2 received a minor injury. The center could not locate the record of injury for Child #2 during the inspection.
Plan of Correction: The center has requested the parent return the original copy of the report so that a copy can be made.
