Inspector Notes
An unannounced monitoring inspection was conducted on 10/02/2023. There were 98 children, ages 6 months- 5 years, 21 staff members providing direct care and supervision and various support staff present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, physical plant, staffing and supervision, programs, emergencies and nutrition. The children were observed during arrival, mealtime, teacher directed activities, self-directed activities and outdoor time. A total of 10 children?s records, 10 staff records and 5 medications were reviewed. The inspector discussed the following with the provider: attendance, personal items, OTC products, and allergies. The inspector arrived for the inspection at 8:30 am and departed at 12:30 pm.
The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Violations
3Evidence: The record for Staff 7 contained documentation that Staff 7 has resided outside of the state of Virginia within the preceding five years. There was not an out of state child abuse and neglect registry check from the state in which the individual has resided in the preceding five years available for review during the inspection for Staff 7. Staff 10 confirmed this was not available for review. The documented hire date in the Record for Staff 7 was 04/03/2023.
Evidence:
1. A spray bottle of surface disinfectant was observed on the shelf over the changing table in Toddler 2 room.
2. A swifter wet jet with the bottle of cleaning materials attached was observed next to the door in Pre-school 1 room.
Evidence: Child 11?s prescription ointment was observed in an unlocked storage tub on a shelf in Toddler 1 room.
