Inspector Notes

An unannounced monitoring inspection, resulting from a self-reported incident received on 01/31/2024, was conducted on 02/06/2024. The inspector reviewed 1 children?s record, 1 staff record, video footage and interviewed staff members. The inspector arrived at approximately 12:00 pm and departed at approximately 1:45 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on video footage, the center failed to ensure that actions towards children that are forbidden by the standards (physical punishment, striking a child, roughly handling or shaking a child) were not used at the center.

In video footage from 01/11/2024 at approximately 11:22 am, Staff 1 was observed closely holding Child 1 while leaning their head down close to the head of Child 1. Staff 1 was observed striking Child 1 twice on the buttocks. Child 1 is observed holding their buttocks after the second strike.
Plan of Correction: Administration has retrained all staff on the definition of forbidden actions and emphasized that it doesn?t matter if actions are playful, they are still considered forbidden.
