An unannounced, on-site focused inspection was initiated on 12/11/2025 and completed on 12/11/2025, as part of an Intensive Plan of Correction (IPOC). The on-site inspection began at 11:15AM and ended at 11:45AM. The inspector reviewed compliance in the areas listed above. This inspection included document review, tour of the facility, interviews, and observations.
The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
